Instructions and forms for applying for studio accounts, grants, scholarships, etc.
FOUNDATION SCHOLARSHIP FUND APPLICATION
Dear Studio Owners, please use the link below to submit a scholarship application on behalf of your students with financial need, (or for yourself).
Our goal is to make funds available to each studio/studio owner to support their students who have financial need in advancing in their development as a martial arts leader.
These scholarships have the following priorities:
- Funds for instructor certification (Jo Kyo, Kyo Sa, Sa Bom).
- Funds to offset membership fees and testing fees.
Scholarship applications are ongoing as long as funds are available.
Please work with your students to prepare answers for the following on-line application questions.
- Type of request – membership dues, testing fees, instructor certification, other.
- Amount requested.
- Briefly describe the student’s need for assistance.
- How does your student intend to support Mission 2000- strengthening human relationships?
- How does the student plan to advance their training after receiving this support?

STUDIO FUNDRAISING ACCOUNT
The Benefit-A-Life Campaign offers the studio a method to provide financial assistance to students to train at its certified studios who have a financial hardship and are unable to pay for the tuition to the studio and to promote community activities for the civic good.
How to complete this form:
Step 1: Download and print a paper copy of the agreement.
Step 2: On the blank lines, clearly write your Studio Name, your Business Address, and your Studio Number.
Step 3: An authorized person (usually the studio owner) needs to sign and date the bottom of the form.
Step 4: Once it is signed, you must send the completed form to the Foundation. You can mail it to:
